Konvoke receives and stores all information that members enter on our website and

of Use. Konvoke collects information on products and vendor market share.
Konvoke reserves the right to collect, disseminate, sell or otherwise disclose non-personal
information provided by its members.
A WORD ON PATIENT INFORMATION:
KONVOKE DOES NOT COLLECT OR SOLICIT ANY PATIENT SPECIFIC INFORMATION. Members should never discuss or provide any patient specific information in their product reviews. Any patient information contained in a review that is identified by Konvoke staff, registered members or any other user will be considered a violation of our privacy policy and removed immediately.
Konvoke does not share, sell, trade or rent your personal information with third parties other than as described within this privacy policy.
We reserve the right to disclose your personally identifiable information as required
by law and when we believe that disclosure is necessary to protect our rights and/or
comply with a judicial proceeding, court order, or legal process served on our Web

Registration
Upon registration, members and vendors are asked to provide both “Public” and
“Anonymous” contact information. Public contact information includes Job
Title and City. Public information will be available for all members and other site
visitors to view.
“Anonymous” (or Private) personal information includes First Name, Last Name, email address, and Hospital name. Private personal information will never be disclosed or shared with other members or visitors to the site unless specifically requested by the member. We use this information to create your online account and allow you to anonymously post your reviews and read the reviews of your peers as well as to send you our monthly newsletters. You may opt-out from receiving our newsletters at any time, please see the “choice/opt-out” section below.
E-mail addresses are used to validate members as hospital-based employees or professionals. We use this to create a vendor-free community of information sharing and user reviews. Your membership and/or participation in our website activities will be canceled without notice if at any time we discover that you provide false information, are not a hospital professional or violate any term of our Privacy Policy or our Terms of Usage Agreement.
Third Party Email Addresses
As a Konvoke member, you will have the benefit of requesting product information directly from any of our participating vendors. When such a request is made personally identifiable contact information be shared with the vendor. In addition, for the purpose of inviting and introducing other hospital professionals to Konvoke, you will be given the option to provide email addresses of individuals to Konvoke. Konvoke will not disseminate or reveal your username or affiliated password to any third party or third party email addresses provided.
Hospital Profile
As a registered member of a specific hospital, you will have the ability to access your hospital’s profile screen and input additional information such as products, software and services currently in use at your facility. Your hospital profile will be available for all members of your hospital to view and edit. Providing information on your hospital profile is entirely optional and can be edited or deleted at any time. Information from your hospital profile will be considered non-confidential and made available to market analysis and other purposes. Hospital names associated with profile information will be kept confidential to all users and non-members of the hospital.
IP Addresses
We receive and store certain types of information whenever you interact with us. When You access or request a page within our site, our Web servers automatically recognize Your domain name and IP address. The domain name and IP address reveal nothing personal about You other than the IP address from which You have accessed the Site.
Cookies
A cookie is a small text file that is stored on a user’s computer for record-keeping purposes. We use cookies on this site. We do not link the information we store in cookies to any personally identifiable information you submit while on our site.

If you would no longer like to receive communications from our mailing list, you can send us an email at the email address above or follow the unsubscribe instructions at the bottom of the newsletter received.
2. HOW IS THE INFORMATION USED?
Konvoke uses the information provided by its members to improve the content and usability of the website in order to make it the most widely used resource for product information. We may periodically contact registered members with special incentives to recommend the use of the site to other hospital professionals. We also may compile non-identifiable information from and about our members and provide to third parties as aggregate information for analysis and other purposes. We will not use or disclose any personally identifiable information about you to other members or third parties, except upon your specific request as described above.
Registered members can contact us directly at privacy@konvoke.com to request any inaccuracies be corrected in a previously submitted review, or to request the addition of a new product, category or vendor. Other users may also contact us to request the correction of any non-review related product inaccuracies, or to request the addition of a new product, category or vendor.
If you choose to use our referral service to recommend our service to your friends, we will ask you for your friend’s name and email address. We will automatically send your friend a one-time email inviting him or her to visit the site. Konvoke stores this information for the sole purpose of sending this one-time email [and tracking the success of our referral program.
Your friend may contact us at privacy@konvoke.com to request that we remove this information from our database.
Konvoke reserves the right to provide personally identifiable information to a third party in response to a verified legal request or compliance with other legal or governmental requirements, investigations and audits.

We use other third parties such as a shipping company to mail giveaways, a credit card processing company to bill featured vendors for services, an email service provider to send out emails on our behalf. Reviews
We allow you to post reviews on the site on products, services or technology that you have experience with. These reviews are anonymous in nature and will not allow users to know what hospital you are from or any personal information about you.
In-mail Tools
If you send in-mails to another member through our site, they will not receive your email address or any personally identifiable information unless you actively choose to share your email address.
Spam
Konvoke does not tolerate spam. We do not sell, rent or otherwise provide your email address to a third party without your specific consent.
Security
We follow generally accepted industry standards to protect the personally identifiable information submitted to us, both during transmission and once we receive it.
